I wasn't sure if Cas even realized she is Dr. Infanta, since nobody used that name, and Shiv didn't really tell him all that much. If Cas doesn't know much about Dr. Infanta to begin with, he might not have made the connection based on the similarity, especially since "treat her as a seven-year-old girl" doesn't exactly scream "top-tier supervillain", and he might have interpreted "a lot older than seven" in terms of years/decades instead of centuries.
Even if he didn't realize, though, now that he knows Alicia personally, I seriously doubt finding out would change his opinion much (if at all).
