Occupational therapists often excel at finding ways to do things, including jerry-rigging tools.
Talking with other disabled folks is another good bet -- my suggestions are based on things I've heard or seen from friends as well as during research for various writing.

This is actually a towel holder, but it gives some ideas how a small strap or slits in a leather add-on could be used to hold a narrow item on a belt.
Belt pencil holders exist -- you'd need to make the handle a tight fit, but this might work with larger ergonomic crochet hooks.
