Conflicting loyalties can make people miserable. There are ways to solve loyalty conflicts and other ethical dilemmas. It's important to know your core values and their order of importance. A good counselor can assist with values clarification and how to use your values to make ethical decisions.
Bonds come in many flavors. The best known -- and at that, they don't know much yet -- in Terramagne seems to be when people with the same superpower connect through it, as Calliope and Vagary did in "Fate Misnamed." But there are other possibilities. In this case, Cas and Ned connected because their souls were clinging to each other in order to keep Ned alive, and you can't do that without forging some kind of link. They already had an affinity strong enough that Ned was able to avoid Cas having traumatic manifestation in "Not a Sudden Problem," so this built on that. And now they have to figure out how to handle it.
Shoulder injuries vary in detail, but much of the treatment is similar. It also works well in a more abstract way for many psychological injuries. A double injury or reinjury happens when someone gets hurt (say, with a partially torn muscle) and doesn't rest enough so they make it worse (say, completing the tear).

Moral injury is one type of personality rupture, painful and difficult if not impossible to repair. I couldn't find anything about other types, which contributes to their prevalence here.

Trust is complicated for abuse survivors. The often feel that they can't trust anyone. This may have been, or still be, correct distrust. Cas has gone from an untrustworthy environment to a more trustworthy one, but his past leaves him struggling to distinguish between untrustworthy and trustworthy people and to believe that help really is available. Other people may not even realize why they have trouble with trust. Some people claim that trust is mandatory, but emotions can't be mandatory. Certainly it is useful in a team and vital for a healthy relationship. But don't try to shame people for not feeling that way. There are ways to learn trust and to gain the trust of trauma survivors.
People often find it hard to ask for help due to many reasons. When you're in over your head, it's a lot better to accept help than fumble through alone. Know how to ask for help and appreciate it. Be ready to help a friend.
