There is a documentary about an Irish set that were conjoined from somewhere above the pelvis down, and it follows the preparation for the surgery to separate the sisters (I saw it '95-'96, maybe TLC?) There is a similar pair somewhere in America, where the parents asked the girls their thoughts and as a result passed on the surgery.
Bruce and Hulk have increasingly marginal life satisfaction if they don't get onto the same page. Hulk cannot see how saving Bruce's life makes him miserable and Bruce can't grok that's what Hulk is doing. He's been gaslight so much that he thinks he deserves the bad things that would happen if Hulk didn't run with the ball.
